Booking Forms These files are Microsoft Word Documents. To Download a File: Right Click over the link below and select "Save Target As...". Then choose where on your computer you would like to save the file. To fill in the forms Online: Left click on the link below and enter your information. Once you have completed the form, you can print the form(s) out and post them to us or you can save the completed form(s) to your computer and then email the forms to krazykaz@kkdisco.co.uk. Please note that your booking request will not be confirmed until we have verified that the date you require is available and the booking fee has been paid. |
